Make sure the iPad is vertical
Visit the website from Safari
Tap share to add the site to Home Screen
Allow 'Camera' from Settings > Safari > Camera > Allow
Turn on Guided Access from Settings > Accessibility > Guided Access Launch the app from Home Screen - then enable Guided Access by triple-clicking the Home button or for iPads with no Home button the Power button.
